5. Past Achievements & Additional Information

Use this section to illustrate your track record. Consider including:

  • A brief highlight of past or ongoing projects that align with ENS, the Web3 domain space, or other relevant activities.
  • Links to GitHub repositories, websites, past proposals, or testimonials.
  • Any relevant data or metrics demonstrating successful deployments, user traction, or meaningful contributions to the ecosystem.
  • If you have any additional documentation, attachments, or clarifications to support your application, include them here or provide links.
  • Mention any notable partners, previous grants, or collaborations that may help the community understand your capabilities and strategic fit.

6. Video Introduction (≤ 5 minutes)

You are required to include a short video introduction:

  • Link to a brief video (5 minutes or less) where your team introduces themselves, your work, and why you believe you can add value to ENS.
Use this video to educate, provide summaries, and showcase your professionalism in delivering on the proposed scopes!
  • Be yourself, make it fun!

7. Conflict Of Interest Statment

List any potential conflicts of interest or participation in any project that is currently receiving from the DAO or is a candidate for it. This list includes (but is not limited to) having any of your team members be a Steward candidate, also being a member of another team who is a candidate for the same stream, having a compensated position at ENS Labs, Karpatkey or any other project who is currently receiving funds from the DAO. Explain how you intend to resolve the conflict if you (and the other party) are selected and how you will have a separation of concerns.

Additionally and optional, please disclose whether your company is currently generating revenue and whether you have participated in any fundraising efforts. Applicants are encouraged to list other active projects they are involved in outside of this application to communicate potential overlaps or conflicts.

Notice that having these (potential) conflicts does not disqualify you from being a candidate, but they must be disclosed, assessed by the delegates voting and addressed if considered an issue. Failing to disclose a conflict or not following through with your proposed solution can lead to a candidacy or even a stream being canceled.

Remember, this template is just a foundation. You are encouraged to add any additional information or sections that might strengthen your application or clarify your value proposition to the DAO. We look forward to reviewing your application’s eligibility and wish you the best of luck in joining the ENS DAO Service Provider Program (Season 2)!
