In an attempt to streamline things for the community, I propose the following format to allow the community to better compare qualifications:

First, do not disclose your politics, religion, or other sensitive matter, unless it would interfere with you doing the job, in which case you self-disqualify.
Second, do not write a book, concise bullet points (no more than a few sentences).
Lastly, do not include irrelevant matter, but whatever you put down must be verifiable.

This is my example— I AM NOT A NOMINEE – I’M RETIRED (according to my wife)

1. Have you served on a Board of Directors or something similar? If yes, short details.
   a. Yes, MercyFirst, a not-for-profit, for four years
2. Communications at the highest level (with whom, general topic, and how often)
   a. Duties of my job required me to regularly interact and negotiate with senior lawyers, senior company D&Os, mediators, judges (at both the state and federal levels), and federal agencies (i.e., SEC and USDOL), pertaining to thousands of cases.
3. Evidence of high-level decision-making, using your independent judgment and discretion:
   a. On behalf of my employer, and for over 20 years, I was the decision-maker pertaining to determining reimbursable loss on D&O, Fiduciary, Employment Practices Liability, and other insurance policies.
   b. On behalf of my employer, and for over 20 years, I was the decision-maker in appointing counsel, incurring and managing defense costs of litigation, including class and mass actions, and litigation brought by regulators.
   c. On behalf of my employer, and for over 20 years, I was the decision-maker on litigation strategy, including whether to settle and at what amount, as well as having significant input on trial strategy on high profile cases.
4. Ability to comprehend and speak on legal matters (you don’t have to be a lawyer).
   a. I possess a law degree from a top university and was admitted to the practice of law in 1995
   b. I was an associate at a top law firm
   c. I actively managed litigation for an insurance company for over 20 years
5. Ability to comprehend and speak on accounting matters (you don’t have to be an accountant)
   a. I possess a BBA in management and accounting from a top university
   b. My job required that I be able to analyze financial statements of Not-for-Profits and Trust Funds.
6. Activity that required the handling of confidential or sensitive information
   a. On behalf of my employer, and for over 20 years, I was involved in sensitive, high-profile litigation, including discrimination, egregious sexual harassment by top level D&Os, and embezzlement and other malfeasance by trustees of union funds.
